Welcome to the website for submissions of research proposals to a NSW Health or Qld Health Human Research Ethics Committee (HREC) and Research Governance Officer/s. This website contains the NHMRC's National Ethics Application Form (NEAF). The Online Forms website also contains the NSW Health and Qld Health Site Specific Assessment (SSA) Forms. For researchers submitting applications within Queensland Health, this website also contains the Qld Health Public Health Act application form (PHA Form).

Before a human research project may commence within the NSW or Qld public health system, it must have undergone the following:

1. Review and approval by an ethical review body, constituted in accordance with the NHMRC National Statement on Ethical Conduct in Human Research (2007), such as an HREC; and
2. Site-specific assessment by each site at which the research is to be conducted.

A research project cannot commence at a site within NSW Health or Qld Health until it has received ethical approval and the SSA has been authorised by the appropriate delegate.
